Anastrozole on TRT: When to Use It and When to Skip It
Anastrozole is an aromatase inhibitor that lowers estradiol. Most men on TRT don't need it. Some absolutely do.
The 80/20
Most men on TRT do not need anastrozole. Estradiol settles into a healthy range on its own. The vocal subset of men who do need it. Usually higher body fat, higher conversion, feel dramatically better once it's added.
How it works
Anastrozole inhibits aromatase, the enzyme that converts testosterone to estradiol. Low doses (0.25–0.5 mg, two to three times per week) are usually enough.
Why low estradiol is also bad
Estradiol is essential for libido, mood, bone density, and even erection quality in men. Crushing it makes things worse, not better. The aim is balance, typically estradiol in the 20–40 pg/mL range (sensitive assay).
