Enclomiphene Dose: Why 12.5 mg Is Usually the Right Starting Place

Lower doses of enclomiphene work for most men, and the difference between 12.5 mg and 25 mg matters more for side effects than for testosterone gains.

The default starting dose

Selvara starts almost every man at 12.5 mg daily. The reason is simple: in trials, the 12.5 mg dose produces 80–90% of the testosterone response of the 25 mg dose with fewer side effects. The smallest effective dose is the right dose.

When to escalate

If, at the six-to-eight-week recheck, total testosterone has not risen meaningfully and symptoms have not improved, the practitioner may move to 25 mg. Most men do not need it.

What the numbers look like

On 12.5 mg, total testosterone typically rises by 100–150% over baseline. On 25 mg, the rise is 130–200%. LH and FSH rise proportionally. Sperm parameters either stay the same or improve.
