Intake & payment

Is the intake actually free? What's the catch?
No catch. No card to start. You only pay if a licensed practitioner prescribes something and you approve the protocol. Anything they don't approve is refunded to your card automatically. If nothing is prescribed, you pay nothing.
When am I charged?
After your intake, you'll see your suggested protocol with transparent pricing. You add a card and approve the line items you want, but we only place a hold. The card isn't actually charged until a licensed Selvara practitioner signs off, usually within 24 hours. You're only ever charged for what ships.
What if the practitioner doesn't approve everything?
Anything they don't approve is refunded to your card automatically. We never charge you for medication that wasn't dispensed.
How long does the intake take?
Average member intake time is 2 minutes 18 seconds. Your goals, symptoms, history. That's it.

Enclomiphene

What is enclomiphene?
An oral medication that signals your body to make more of its own testosterone. No injections, fertility preserved. The lowest-friction protocol for most men under 45.
Is enclomiphene legal without a TRT clinic?
Yes. Enclomiphene is a non-controlled prescription medication. A licensed Selvara practitioner reviews your intake and prescribes when clinically appropriate.
Do I need bloodwork to start enclomiphene?
Not to begin the intake. You can start without any prior labs. If your practitioner decides bloodwork would help refine your protocol, they'll order it through our partner lab.
How fast will I feel a difference?
Phase II research showed average testosterone rising from 209 to 604 ng/dL in 6 weeks on 25mg. Most members report noticing energy changes around week 4–6.

TRT (testosterone)

Do I need bloodwork before TRT?
Usually, yes. TRT is dosed off your baseline labs. If you have recent comprehensive bloodwork, upload it. If not, your practitioner will order it through our partner lab as part of your protocol.
Injection vs. cream TRT?
Weekly cypionate injection is the most common protocol. Steady levels, lowest cost. Cream is a daily topical option for members who prefer not to inject.
What if I want to preserve fertility on TRT?
Enclomiphene is our standalone fertility-preserving option. Men who want testosterone support without shutting down endogenous production typically start with Enclomiphene 12.5 mg or 25 mg before considering injectable TRT.
How are follow-up labs handled?
Standard cadence is baseline, month 3, and month 6, then annually. Your practitioner adjusts dose based on each panel.

GLP-1 (weight loss)

Tirzepatide or Semaglutide. Which one?
Tirzepatide ($249) is a dual GLP-1 / GIP agonist with stronger weight loss data, around 20.9% body weight loss at 72 weeks. Semaglutide ($189) is a GLP-1 only agonist with around 15% loss. Your practitioner picks based on your goals, side-effect profile, and history.
Will I lose muscle on a GLP-1?
Without resistance training and protein, you can. We pair every GLP-1 protocol with a Muscle Preserve supplement pack and a written training note from your practitioner so the weight you lose is fat, not lean mass.
How long do men stay on a GLP-1?
Most members run a 6 to 12 month protocol, then taper to a maintenance dose or cycle off. Practitioners review every 90 days.

Peptides (Sermorelin)

What does Sermorelin actually do?
Sermorelin is a growth-hormone-releasing peptide. Members report deeper sleep, faster recovery from training, and gradual lean-mass gains. Published data shows roughly 39% increase in IGF-1 over a 12 to 16 week course.
How is it taken?
Nightly subcutaneous injection, 30 minutes before bed, on an empty stomach. We ship insulin-style syringes and a written 8-step guide with every kit.
Can I stack it with TRT or a GLP-1?
Yes. Sermorelin pairs especially well with TRT for recovery and with a GLP-1 for muscle preservation. Your practitioner reviews the full stack.

Longevity (NAD+, Metformin, LDN)

How often is NAD+ injected?
2x weekly subcutaneous, dose titrated to tolerance. Most men notice cleaner energy and mental clarity inside 3 to 4 weeks. NAD+ declines by roughly 50% from your 20s to your 50s, which is why supplementation is the centerpiece of the longevity stack.
Why Metformin if I'm not diabetic?
Off-label longevity use. The published TAME and MILES trials suggest Metformin improves insulin sensitivity, slows mitochondrial decline, and may extend healthspan. Dose is 500 mg to 1000 mg daily, titrated by your practitioner.
What is Low-dose Naltrexone for?
LDN at 4.5 mg nightly has emerging data for inflammation, sleep quality, and recovery. It is off-label and dispensed only when your practitioner determines it is clinically appropriate.
